Grand Peaks Medical, Dental & Behavioral Health is a multi-specialty, non-profit Community Health Center with locations in St. Anthony and Rexburg.  We offer affordable quality healthcare and are able to offer discounts based on income and household size.  Our patient’s health and well-being is in the hands of experienced primary care Providers. We are dedicated to providing quality primary and preventive healthcare in the Upper Valley communities of Eastern Idaho.
